Phimosis

What exactly is a Phimosis?

Standard Coverage

  • Under normal circumstances, the foreskin should be able to retract over the glans or head of the penis especially in a flaccid state
  • A phimosis is a condition in men where the foreskin cannot retract fully over the glans of the penis
  • In some cases it is impossible for the foreskin to slide back over the glans or the head of the penis even in a flaccid state
  • It might be present since early childhood although a small percentage of males develop a phimosis in adulthood.
  • With every erection or during intercourse when the foreskin is stretched, small linear tears occur in the foreskin
  • This in turn creates scar tissue in the foreskin and the phimosis becomes tighter and tighter
  • Every time this happens the scar tissue rips and more scar tissue forms, increasing the tightness but also the skin itself

Why should it be corrected?

  • Because of the fibrotic nature of the development, the phimosis wont recover and might only get worse over time
  • It can lead to problems with hygiene:
  • Can’t clean behind the foreskin.
  • Smegma (sebaceous secretions) accumulating under the skin
  • Foul/potent odours from the penis
  • Urine/semen accumulating under the foreskin
  • Bacteria and fungi can culture under the foreskin
  • An irritable itch
  • Acute or chronic balanitis, an infective, inflammatory disease of the foreskin and shaft which can be precancerous
  • Possibility to transfer infections to partners during intercourse
  • Psychological implications affecting your sex life or intimate relationship with your partner
  • Risk of a paraphimosis, a surgical emergency:
  • The foreskin pulls back behind the glans or head of the penis but is too tight to pull over again.
  • This leads to reduced blood supply to the glans and is extremely painful. The glans swells up and if left untreated could lead to necrosis of the glans
  • This might require emergency surgical correction
  • Development of BXO

What is BXO?

BXO or Balanitis Xerotica Obliterans is a chronic, sometimes progressive disease which can lead to phimosis or urethral stenosis (narrowing of the opening of the urethra). It can affect both urinary and sexual function and might lead to Squamous Cell Carcinoma, a form of penile cancer.
Balanitis – recurrent chronic and acute infections of the glans and under the foreskin
Xerotica – White skin or loss of the normal color of the skin – the skin takes on the appearance of scarred skin with the loss of the normal light to dark brown color of the foreskin
Obliterans – Obliterating – closing up or tightening of the foreskin

I’m now 2 weeks post-op and happier than ever with the results

So I had suffered from Phimosis and only up until recently did read up about what it is via the website that is linked with Arte Novi Clinic. I booked a consultation for a Friday afternoon to find out more about the treatment involved and about recovery etc. I left the clinic and arranged the following week to have the treatment done. One of my main concerns was when I could resume work and return to exercising. The procedure we had agreed to do was Partial Circumcision and Frenuloplasty. When I went to the clinic I received a local anaesthetic which made me feel like I dozed off for 5 mins however 1-hour passed and the procedure was done! No pain at all, only a little nauseous that evening from the local anaesthetic which is a common side effect that lasts only about 20 seconds. I had no complications and by Tuesday morning I was able to resume to Spinning classes with zero pain or discomfort! The stitches dissolved within 7 – 10 days and I’m now 2 weeks post-op and happier than ever with the results. Dr Sam was always accessible through call or text with any concerns I had day or night. Outstanding

Got the partial circumcision from doctor Sam and I have to say that the service was outstanding. Initially, I sent an email explaining my query and Sam got back to me super fast and even offered me a phone call to discuss everything fully. I was provided with a date for my appointment within days and had the op done in no time. Sam and his receptionist are really nice and genuine people who are really easy to talk to which makes you feel really comfortable on the day of the operation. The operation was painless and the recovery was far easier than I had anticipated. I was even provided with a free meal afterwards. My only regret is that I didn’t get the operation sooner. “The procedure was quick and painless”

I got a circumcision done by Dr Sam in Feb 2019 after many years of suffering. My only regret is not finding Dr Sam sooner. He was world-class. Very calm and reassuring. He talked with me on the phone prior to surgery and any questions he was on hand to answer as was his secretary. On the day of the surgery he made the whole process so straightforward I was in and out in no time. The procedure was quick and painless.
